The Following Items Are Required for Your Application:
    1. One recent passport style color photo, with full face, front view, no hat, and against a plain light background.

    2. Original passport that is valid for at least 6 months.

    3. Visa application form of the People's Republic of China.

        IMPORTANT NOTICE :        As of July 19, 2011, the Visa and Passport Office of the Chinese
         Consulate General in New York will require all applicants to use the 2011 edition of the "Visa Application
         Form of the People's Republic of China". The Visa and Passport Office would like to bring applicants'
         attention to the following:

       
    A. The Visa Office only accepts the “Visa Application Form of the People’s Republic of ” (Form V.2011A) marked “CNNYCONSULATE JUNE 2011 EDITION” downloaded from the website of the Chinese Consulate in . Previous editions or forms downloaded from other sources will not be accepted.
    B. Please consult the “Instructions on the Completion of the Visa Application Form” while completing the application.  All sections must be filled out truthfully on a computer. If a section does not apply to you, please state “N/A”. Incompletely filled-out forms, hand-written or altered forms will not be accepted.
    C. The completed form should be clearly and completely printed out on letter sized papers. Please use two sided printing if possible.
    D. All applicants must sign the application form even if the form is completed by another person, except that a parent or guardian may sign on behalf of an applicant under the age of 18.
    E. If you are applying for work visa or student visa, or if you are not a US citizen, or if you will be accompanied by anyone who shares the same passport with you, you also need to complete the Supplementary Application Form V. 2011B.

    4. When apply for visa for the first time, applicants who used to own passports that were issued by People's     Republic of China, but are now naturalized U.S. citizens, are required to present the original official Chinese     passport.

    5. When apply for visa for the first time, children/youth under 18 years of age, are required to present a copy of     the birth certificate along with father's or mother's U.S. passport or Chinese passport, or copy of the green
        card. A photo copy is also required for old visa that has passed its validation date.

    6. New U.S. passport holders are required to present the original U.S. passport if already visited China and the     passport bearing seal of Chinese visa.

The Passport and Visa Office will no longer accept cash payment. Only the following will be accepted:
        1. Master Card, Visa Card Only.
        2. Money Order or Company Check (no Personal Check). Payable to Chinese Consulate General.
